How do I submit a Qualified Domestic Relations Order (QDRO) request?

Participants and the "alternate payee(s)" are encouraged to consult a legal advisor when preparing a domestic relations order (DRO).

When you are in the process of submitting a Qualified Domestic Relations Order (QDRO) request, please be sure to contact our participant support team to receive a QDRO Determination Procedure document. This document will outline the QDRO process and highlight the items you’ll want to ensure are included in your court order. The Summary Plan Description has general information on your plan for you to review and can be found within the Your Plan Information folder of your participant dashboard. 

In addition, you may upload a draft of the order to the Shared Files folder of your participant dashboard and notify us for preliminary review. 

If the order conforms to the requirements of ERISA, the Internal Revenue Code, and the Plan's QDRO rules and procedures, then we can typically review within 30 business days of receipt. If the order does not meet these requirements, you may continue to draft and submit amended orders until a qualified order is provided.

Once you have received a certified copy of the court order, be sure to mail the court-issued DRO to our headquarters for final review:
